Q: What do I need to bring for my first visit?
A: First time patients must bring identification and insurance information. If under 18 a legal guardian must be present with you.
Q: Can I fill out the medical forms prior to my appointment with Bullis Orthopedics & Sports Medicine?
A: Yes, please go to Patient Forms on our website. If the forms are complete upon arrive, please arrive 10-15 minutes early. If the forms are not completed, please arrive 30-45 minutes early.
Q: Do I need to be early for my scheduled appointment time?
A: Yes, 10 to 15 minutes. Please see above question.
Q: What information should I bring for the doctor?
A: Bring a detailed list of medications, past medical history, doctor’s notes, x-rays, and/or MRI’s.
Q: Am I responsible for a payment at the time of my visit?
A: Yes, you are responsible for co-pays and deductibles
Q: What insurance do you accept?
A: We currently accept Medicare, Medicaid and several HMO/PPO organizations. Please call us to obtain a current list of the participating carriers.
Q: What are your office hours?
A: We are open Monday-Thursday from 8:00 a.m. to 5:00 p.m. and on Friday from 8:00 a.m. to 4:30 p.m.
Q: Do I need a referral?
A: Please check with your primary care physician.
Q: What ages do you treat?
A: We service all age groups.
Q: Is parking available?
A: Onsite parking is available in the rear of the building off Prospect Street.
Q: What languages are spoken?
A: We have Spanish and Portuguese speaking staff.
Q: What happens if I need an X-Ray or an MRI?
A: We have X ray services on site and MRI services available nearby.
Q: Will I need to have follow up care for rehabilitation?
A: It is a good possibility that you may need restorative therapies. Bullis Orthopedics has physical and occupational therapy nearby.
